What You'll Do11
- 1Conduct initial and ongoing assessments to evaluate mobility coordination strength and current level of function
- 2Administer evaluate and interpret tests to determine functional goals and outcomes
- 3Provide written evaluations and care plans outlining physical therapy needs progress and expected results
- 4Deliver therapy services adhering to physician orders while maintaining accurate documentation
- 5Modify treatment plans based on client performance and collaborate with healthcare providers
- 6Lead discharge planning educate caregivers on home exercise regimens and coordinate multidisciplinary services
- 7Supervise PTA and home health aide personnel as needed
- 8Complete OASIS assessments where permitted by state law
- 9Serve as a resource consultant for clinical staff
- 10Ensure all documentation meets payer review standards without technical or clinical denials
- 11Maintain effective communication among staff family members and external partners
Requirements5
- 1Degree in Physical Therapy from an accredited institution
- 2Current unrestricted state license as a Physical Therapist
- 3Valid CPR certification
- 4Experience in Home Health Therapy and OASIS documentation
- 55+ years of clinical practice
