Physical and Occupational Therapists - Home Visits
Overview
BrightStar Care seeks a compassionate PT OT to deliver home visits across Baltimore County. You will conduct assessments develop treatment plans modify home environments and coordinate care to empower clients to live independently. What You'll Do9
- 1Conduct comprehensive assessments to evaluate clients functional abilities limitations and therapy needs
- 2Develop and implement individualized treatment plans in collaboration with physicians and other healthcare professionals
- 3Assess home environments and recommend modifications to improve safety reduce fall risks and enhance independence
- 4Identify adaptive equipment needs and provide recommendations to maximize client function and quality of life
- 5Monitor client progress adjust treatment plans as needed and communicate changes to the Director of Nursing and physicians
- 6Coordinate care with interdisciplinary team members caregivers and community resources to ensure continuity of care
- 7Educate clients families and caregivers on therapeutic interventions adaptive techniques and safety strategies
- 8Prepare accurate clinical documentation progress reports treatment summaries and discharge plans promptly
- 9Maintain compliance with professional regulatory and agency standards while delivering patient centered care
Requirements10
- 15+ years building ETL pipelines with Spark and Airflow
- 2CPC Card certification
- 3BCBA credential
- 4AWS Certified experience
- 55+ years building ETL pipelines with Spark and Airflow
- 63-5 seasons of professional experience
- 7Strong clinical assessment and treatment planning skills
- 8Ability to work independently and effectively manage time and caseloads
- 9Excellent communication interpersonal organizational and problem solving abilities
- 10Experience with electronic medical records preferred
