What You'll Do11
- 1Conduct outpatient visits across designated neighborhoods providing hands-on therapy sessions
- 2Perform comprehensive mobility assessments including range of motion strength and balance evaluations
- 3Develop customized rehabilitation strategies using manual techniques gait training and therapeutic exercises
- 4Maintain detailed clinical documentation using advanced digital tools
- 5Coordinate with nursing staff to integrate physical therapy recommendations into overall care plans
- 6Educate patients and families on therapeutic exercises and home safety modifications
- 7Analyze patient progress and adjust treatment approaches based on measurable outcomes
- 8Collaborate with multidisciplinary teams to ensure holistic care delivery

Requirements9
- 1Active Maryland Physical Therapist license with current certification
- 2Minimum 5+ years of clinical experience in outpatient physical therapy
- 3Proven ability to work autonomously in diverse environments
- 4Strong communication skills for patient education and interdisciplinary collaboration
- 5Familiarity with electronic health record systems demonstrated through prior employment
