Overview

You own the patient journey from first call through ongoing care at Keta Medical Center, a growing network of mental health clinics across the New York metro area. You bridge patients, families, and clinical teams across multiple sites, ensuring every patient feels guided and heard during treatment. You work alongside physicians, nurses, and therapists to support retention and outcomes. This role stands out for its depth: you build long-term relationships with patients over months and years, not just one-off interactions.

What You'll Do9

  • 1Serve as the primary point of contact for new and existing patients, answering questions and providing clear, empathetic information about treatment and scheduling.
  • 2Greet patients at first visits and conduct comprehensive orientations that review program structure, introduce clinic staff and resources, and outline what to expect during each phase of care.
  • 3Educate patients on available programs, psychiatric services, therapy modalities, and treatment options to support informed decisions.
  • 4Track patient progress throughout the entire care duration, proactively following up on missed or delayed sessions and identifying barriers to engagement.
  • 5Assist with care coordination, therapist assignments, and scheduling logistics across programs.
  • 6Collaborate with clinical teams including physicians, nurses, and therapists to support patients needing extra attention and coordinate individualized care plans.
  • 7Communicate patient needs and concerns to clinical staff promptly, ensuring relevant updates are shared across the care team for continuity.
  • 8Attend regular team meetings to review upcoming sessions, debrief on completed treatments, and align on patient readiness and clinical factors.
  • 9Participate in community-based events to represent the practice, build awareness of services, and support outreach efforts.

Requirements9

  • 1Excellent communication and organizational skills, with a compassionate, proactive approach to patient care.
  • 2Experience in healthcare, customer service, or mental health settings preferred.
  • 3Ability to manage details while building strong, supportive relationships with patients.
  • 4Support patient commitment to and understanding of the treatment process.
  • 5Bachelor’s Degree in Social Work, Public Health, Psychology, Healthcare Administration, or a related field preferred.
  • 6Familiarity with EHR/CRM systems such as Salesforce.
  • 7Strong data-tracking skills with spreadsheets or dashboards to monitor patient metrics.
  • 8Community outreach experience, preferably in a mental health setting.
  • 9Willingness to travel to multiple clinic locations across the New York metro area.
