Overview
The Patient Admission Officer (PAO) oversees screening admissions and transfers for Advanced Care at Home. This role balances capacity management with clinical command center oversight. You will coordinate admissions based on real-time discharge flow and maintain communication with frontline clinicians. This position offers unique exposure to both hospital and home-based care environments.
What You'll Do11
- 1Maintain continuous contact with the Advanced Care at Home clinical command center and Administrator on Call to monitor capacity and respond to patient requests
- 2Review daily triage Epic lists and EADS model lists to identify eligible patients for admission or transfer
- 3Analyze medical records to assess eligibility and track potential patient pipeline
- 4Collaborate with brick and mortar facility clinicians to ensure appropriate patient selection
- 5Coordinate scheduling with Service Coordinators and Paramedics for admission times
- 6Act as primary contact for external stakeholders regarding patient admission inquiries
- 7Escalate clinical appropriateness questions to Clinical Nurse Manager or Medical Director
- 8Provide referrals to Utilization Management nurses and Physician Advisors for level of care assignments
- 9Communicate screening requirements to virtual hospitalists and Mission Control RNs
- 10Troubleshoot conflicts and manage challenging situations professionally
- 11Participate in daily multidisciplinary team meetings and weekly operational reviews
Requirements8
- 1Associates degree in Nursing from a state accredited school with BSN preferred
- 2Current Colorado RN license and certification such as CPC Card or BCBA
- 3Five years of nursing experience in tertiary care settings with one year managing patient flow
- 4Proven ability to work in fast paced environment with flexible schedule
- 5Strong communication and collaboration skills across disciplines
- 6Critical thinking abilities to prioritize complex patient cases effectively
- 7Experience with Advanced Care at Home operations and ED triage systems
- 8Ability to manage multiple priorities while maintaining accuracy under pressure
