- 1Develop and maintain clinical protocols policies and procedures while collecting data for CQI programs
- 2Drive patient care outcomes through skilled nursing interventions and protocol adherence
- 3Lead multidisciplinary collaboration to provide comprehensive HIV/AIDS care
- 4Provide direct nursing services including phlebotomy immunizations and health screenings
- 5Manage patient triage and messaging systems ensuring timely responses
- 6Prepare examination rooms and enforce infection control measures
- 7Document all care activities in electronic medical records following agency policies
- 8Coordinate pharmacy exchanges and process patient referrals for specialty care
- 9Educate patients on HIV management adherence and prevention strategies
- 10Participate in CQA committees and prepare statistical reports as needed
- 11Contribute to positive work environment fostering cultural competency and diversity
Requirements7
- 1Active LPN license with CPR/BLS certification
- 2Three years or more experience in medical settings
- 3Proficiency in Microsoft Office applications
- 4Reliable transportation and driver's license
- 5Strong written and verbal communication skills
- 6Thorough knowledge of HIV/AIDS patient care and cultural sensitivity
- 7Experience with health social service needs of diverse populations
