Overview
The Ambulatory Clinic Licensed Practical Nurse (LPN) will lead patient care coordination at Hedge Lane Medical Pavilion. This full-time position requires a licensed LPN with strong clinical skills to deliver comprehensive care within the Kansas Nurse Practice Act guidelines. You will collaborate with physicians and multidisciplinary teams to improve patient outcomes while supporting interdisciplinary approaches.
What You'll Do15
- 1Greet patients and prepare them for examinations or procedures
- 2Record health histories and document abnormal findings for physicians
- 3Take vital signs and conduct required screenings
- 4Administer medications safely including IM subcutaneous and oral routes
- 5Observe and report patient conditions and drug reactions
- 6Prepare supplies and assist physicians or APPs during treatments
- 7Maintain accurate patient charts and forward documentation
- 8Schedule tests and coordinate referrals with provider authorization
- 9Collect and deliver lab specimens while entering results into the EMR
- 10Notify patients of test results and schedule follow-up appointments
- 11Provide patient education on medications and treatment plans
- 12Review medications and provide post-visit instructions
- 13Handle telephone calls for referrals and coordinate pre-visit planning
- 14Complete necessary forms such as FMLA and Home Health orders
- 15Perform professional clinical competencies as required by unit needs
Requirements10
- 1Licensed Practical Nurse graduate from accredited program
- 2High school diploma or GED
- 3Basic computer skills required
- 4Current BLS or BCLS certification from AHA
- 5Multi-state LPN license from Kansas or Missouri
- 63-5 years clinical experience preferred
- 7Knowledge of Spark and Airflow for ETL pipeline development
- 8Valid Kansas or Missouri LPN license
- 9Ability to perform tasks within defined scope of practice
- 10Commitment to patient-centered care standards
