What You'll Do11
- 1Coordinate care for assigned patients and deliver expert physical and psychosocial assessments
- 2Ensure specialist referrals and treatment plan adherence through physician coordination
- 3Assist families in accessing community resources to improve health outcomes
- 4Educate patients and families on disease processes and treatment options
- 5Provide emotional support and counseling during illness transitions
- 6Evaluate patient recovery using appropriate clinical resources
- 7Empower patients through goal-setting and advance care planning discussions
- 8Partner with clinicians across settings to manage care transitions
- 9Facilitate discharge planning to reduce readmissions and improve outcomes
- 10Maintain patient confidentiality and protect privacy rights
- 11Collaborate with multidisciplinary teams to optimize patient experiences
Requirements10
- 1Minimum 2 years registered nursing experience in community-based settings
- 21-2 years nursing experience in palliative/hospice care preferred
- 3Knowledge of interdisciplinary team dynamics
- 4Strong patient education and communication abilities
- 5Valid RN license in Florida with current state certification
- 6Bilingual proficiency in English and Spanish
- 7Reliable transportation with valid driver's license
- 8Bachelor's degree in nursing preferred
- 9BLS certification required
- 10Experience with electronic health records and documentation systems
