Overview

Matrix Medical Network seeks a licensed Nurse Practitioner to perform in-home Comprehensive Health Assessments for adults and seniors, delivering personalized care at scale in patients' residences. You will assess medical history, medications, and social factors, order point-of-care tests, and coordinate with PCPs and care teams. This role offers flexible scheduling, travel opportunities, and potential transition to full-time roles.

What You'll Do10

  • 1Build and perform adult and geriatric health assessments in patients' homes or facilities
  • 2Document findings and recommendations with clear, compliant notes according to Matrix protocols
  • 3Administer point-of-care and lab-based testing as needed to inform care plans
  • 4Coordinate with physicians, case managers, social workers and family caregivers to close care gaps
  • 5Educate patients and families on health maintenance and follow-up care, including referrals
  • 6Collaborate with PCPs on patient education and follow up to ensure continuity of care
  • 7Provide services at patient residences including homes and Skilled Nursing Facilities
  • 8Maintain accurate records and ensure compliance with Matrix guidelines and privacy regulations
  • 9Ensure safe, timely scheduling of visits and efficient travel arrangements
  • 10Remain current with licensure, certifications and ongoing professional education

Requirements10

  • 1Current NP licensure in state of practice with prescriptive authority or eligibility
  • 2AANP AACN or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care
  • 3BLS and ACLS or CPR certification
  • 41 year of NP experience
  • 5Ability to travel up to 80-90% of the time including overnight travel and weekends
  • 6Valid state driver license and automobile insurance coverage for state of residence
  • 7Strong computer skills and familiarity with health/medical record software
  • 8Excellent verbal and written communication skills
  • 9Commitment to patient-centered care and teamwork
  • 10Eligibility for additional state licensure as needed and support for credentialing efforts
