What You'll Do11
- 1Provide comprehensive primary care to residents in skilled nursing long-term care and assisted living settings
- 2Own patient panels and drive treatment plans
- 3Collaborate with nursing staff therapists and physicians to coordinate care
- 4Utilize Electronic Health Records to document and manage patient information
- 5Ship follow-up appointments and coordinate referrals
- 6Maintain compliance with Missouri APRN licensing and collaborative practice regulations
- 7Ensure accurate Medicare PECOS and Medicaid enrollment documentation
- 8Develop care plans aligned with CMS skilled nursing facility regulations
- 9Implement value-based care strategies within ACO frameworks
- 10Engage in geriatric prescribing principles and point-of-care nursing initiatives
- 11Contribute to a culture of clinical autonomy and physician partnership
Requirements11
- 1Master's degree or higher from an accredited Nurse Practitioner program
- 2National board certification in Family Practice Adult Skilled Nursing or Geriatrics
- 3Active Missouri APRN and RN licenses
- 4Eligibility to practice under Missouri Collaborative Practice Arrangement
- 5DEA and Missouri BNDD registration or ability to obtain
- 6Medicare PECOS enrollment and Medicaid eligibility
- 7Valid driver's license with reliable transportation
- 8Two or more years of Nurse Practitioner experience
- 9Proficiency with PointClickCare or similar long-term care EHR systems
- 10Understanding of CMS skilled nursing facility regulations
- 11Experience with value-based care models and ACO structures
