Owns patient care in the home for adults and children with complex medical needs including trach and vent support at scale across North Carolina. What You'll Do10
- 1Build individualized care plans for patients with complex medical needs including trach and vent support
- 2Provide high quality in-home nursing care and family coaching across ages
- 3Maintain safety by thorough home intake and case selection aligned with comfort level
- 4Collaborate with clinical team for continuous training and skill growth in trach/vent care
- 5Deliver compassionate care with consistent communication to families and care team
- 6Support scheduling flexibility by selecting cases and shifts
- 7Ship patient progress notes and care outcomes to the clinical team
- 8Coordinate with 24/7 support for urgent clinical questions
- 9Ensure compliance with CPR, TB status, and licensure requirements
- 10Demonstrate safety first culture in every home visit
Requirements10
- 1Current LPN or RN license in North Carolina or a compact state
- 2Current CPR certification (in-person only)
- 3TB skin test completed within the past 12 months
- 4Ability to provide care across ages including pediatric patients
- 5Willingness to learn and train for trach/vent care
- 6Excellent communication and family coaching skills
- 7Commitment to safety and high-quality care
- 8Ability to work onsite in Raleigh, North Carolina
- 9Flexible scheduling availability
