Mobile Nurse Practitioner, Home Health in San Gabriel Valley
Overview
Own home-based care delivery at scale as a Mobile Nurse Practitioner serving the San Gabriel Valley. You’ll autonomously assess, treat, and manage medically complex and recently discharged patients in their homes while collaborating with an interdisciplinary team. You’ll build lasting patient relationships, reduce hospitalizations, and support safe recoveries at home. This role blends clinical autonomy with strong team support and a focus on relationship-centered care.
What You'll Do8
- 1Build comprehensive home-based care plans for acute, post-acute, and transitional patients
- 2Diagnose and treat medical conditions in patients' homes and adjust plans as needed
- 3Coordinate follow-up care with physicians, nurses, pharmacists, social workers, and care managers
- 4Educate patients and families on disease management and preventive care
- 5Identify complications early and intervene to prevent hospitalizations
- 6Maintain accurate clinical documentation and communicate during transitions with hospitals and facilities
- 7Partner with an interdisciplinary team to deliver high-quality, patient-centered care
- 8Drive independence in field-based practice while ensuring safety and quality
Requirements10
- 1MSN in Nursing
- 2Adult/Geriatric Nurse Practitioner certification
- 3Active, unrestricted RN license in California
- 4Active, unrestricted NP license in California
- 5Minimum of 1 year NP clinical experience
- 6Strong assessment, diagnostic, and care management skills
- 7Excellent communication and interpersonal skills
- 8Ability to work independently in a field-based role
- 9Valid driver’s license and reliable transportation
- 10Current TB test or willingness to obtain
