Medical Nutrition Coordinator, Enteral Home Care
Overview
You own patient followup for enteral therapy, evaluating adherence to physician orders and driving outcomes across the Dallas area. You educate patients on enteral nutrition protocols, manage feeding equipment like feeding pumps and IV poles, and coordinate with referral sources to secure insurance coverage. You report to a clinical supervisor and collaborate with nurses, dietitians, and physicians. This role combines hands-on patient care with operational follow-through, and you will directly shape patient recovery through consistent education and equipment delivery.
What You'll Do10
- 1Evaluate patient adherence to physician orders during routine followups, adjusting education to close gaps in enteral nutrition compliance.
- 2Deliver and set up feeding pumps, IV poles, and formula supplies in patient homes, verifying safe operation and proper use.
- 3Conduct complete patient assessments per policy, documenting findings in records that meet JCAHO accreditation standards.
- 4Gather insurance details from referral sources, submitting paperwork to guarantee coverage for every enteral patient.
- 5Follow up on missing documentation and claims to secure payment for all services rendered.
- 6Educate patients and caregivers on enteral feeding techniques, addressing questions and reinforcing physician orders.
- 7Drive to patient locations, managing routes to maximize visits while maintaining a professional appearance and positive company image.
- 8Maintain precise patient records, logging each interaction and equipment check to support continuity of care.
- 9Participate in ongoing training, including in-services on new enteral therapy methods, to keep skills current.
- 10Represent Lincare in written reports and patient interactions, building trust with doctors and referral partners.
Requirements9
- 11+ year in home health care, hospital, clinic, or nursing home setting preferred.
- 21+ year of experience as an LPN, DTR, or CNA preferred.
- 3Education or experience in dietetics, nutrition, home health, or nursing strongly preferred.
- 4Read and interpret documents such as safety rules, operating instructions, and procedure manuals.
- 5Calculate figures and amounts including proportions, percentages, area, and volume.
- 6Solve practical problems with limited standardization and interpret written, oral, diagram, or schedule instructions.
- 7Use word processing and spreadsheet software plus email for documentation and communication.
- 8Lift and move up to 50 pounds during equipment delivery.
- 9Excel at human relations to interface with doctors, referral sources, and patients.
