What You'll Do8
- 1Provide skilled nursing care to pediatric and adult patients in their homes, following the plan of care and documenting with the tablet-based system.
- 2Administer medications, treatments, and therapies as ordered, monitoring patient response and adjusting care in collaboration with the care team.
- 3Assess patient condition, recognizing changes and reporting to the supervising clinician or physician to ensure optimal outcomes.
- 4Educate patients and families on care plans, disease management, and medication use, fostering confidence and independence.
- 5Coordinate with interdisciplinary team members, including nurses, therapists, and physicians, to ensure continuity of care.
- 6Maintain accurate and timely electronic documentation of all nursing interventions and patient responses.
- 7Adhere to infection control and safety protocols, including proper use of PPE and compliance with vaccination requirements.
- 8Respond to patient emergencies, providing immediate care and activating emergency services as needed.
Requirements8
- 1Active, unencumbered LPN/LVN license in the state of Missouri (compact licenses must transfer within 60 days).
- 2Current CPR certification with hands-on component (Aveanna can assist after hire if needed).
- 3TB skin test within the last 12 months.
- 4Reliable transportation to travel to patient homes.
- 5Experience in homecare, pediatrics, or a similar setting is preferred but not required.
- 6Ability to work flexible schedules, including days, nights, weekends, and holidays.
- 7Must comply with all health-related requirements, including vaccinations and testing, subject to exemptions.
- 8Strong clinical assessment and critical thinking skills, with a compassionate, patient-centered approach.
