Licensed Practical Nurse (LPN) - Private Duty Homecare
Overview
Deliver skilled nursing care to pediatric and adult patients in their homes across Denver and Aurora. As a LPN on our private duty team, you will own clinical outcomes for 1-2 patients per shift, documenting via electronic. This contract role includes flexible scheduling, CPR certification, and paid training. Partner with a 24/7 local support team and advance through skill-based career paths in one of the nation's largest homecare providers.
What You'll Do8
- 1Own patient assessments and medication administration for assigned cases, documenting via electronic tools in the Aveanna tablet.
- 2Implement physician-ordered care plans, adjusting for patient response and reporting changes to the supervising RN.
- 3Coordinate with families and interdisciplinary team members to ensure continuity of care across day, night, and weekend shifts.
- 4Administer IV therapy, g-tube feedings, and tracheostomy care, following competency validation.
- 5Educate patients and caregivers on disease management, medication side effects, and safety protocols.
- 6Maintain accuracy of electronic health records, documenting interventions within 24 hours.
- 7Drive to multiple patient homes per shift, adapting to changing schedules and case assignments.
- 8Respond to emergency situations per CPR training and escalate to clinical leadership.
Requirements6
- 1Active, unencumbered LPN license in Colorado.
- 2Current CPR certification with hands-on component.
- 3TB skin test within last 12 months.
- 46+ months hands-on nursing experience preferred.
- 5Reliable transportation for home visits.
- 6Able to work Mon, Tue, Fri days for urgent case.
