Licensed Practical Nurse (LPN) - Homecare
Overview
You will provide skilled nursing care in private homes for pediatric and adult patients across Denver, managing caseloads of 1-3 patients per shift. You will report to a Registered Nurse and collaborate with a 24/7 clinical support team using Aveanna's tablet for electronic documentation. This role offers a $3,500 sign-on bonus for full-time nights and $2,500 for days, plus flexible scheduling with AM and PM shifts. What You'll Do8
- 1Deliver and coordinate quality patient care, following physician orders and the individual care plan for each patient.
- 2Assess patient condition and care needs continuously, reporting changes to the supervisor or physician within 15 minutes.
- 3Document all activities, assessments, and nursing actions in Aveanna's tablet before the end of each shift.
- 4Participate in creating and updating the nursing care plan, adjusting interventions based on patient response.
- 5Implement infection control measures per OSHA standards to protect staff and patients.
- 6Educate patients and families on disease process, self-care, and prevention strategies.
- 7Maintain competency through continuing education and internal skill evaluations.
- 8Coordinate with the RN supervisor and local clinical team to ensure seamless homecare delivery.
Requirements6
- 1Current, unrestricted LPN license in Colorado
- 2CPR certification with hands-on component (Aveanna can assist after hire)
- 3TB test current within 12 months
- 46 months prior hands-on nursing experience, preferred
- 5Reliable transportation required
- 6Graduate of an accredited school of nursing
