What You'll Do7
- 1Build and deliver direct patient care following the plan of care under RN supervision
- 2Provide accurate and timely documentation aligned with the plan of care
- 3Educate patients and families on self-care techniques and diagnosis related information
- 4Coordinate home health services with the broader care team and report needs to RN or Clinic Manager
- 5Prepare and use equipment and supplies in a safe, aseptic manner
- 6Participate in ongoing professional growth and development
- 7Support hospice patients as census allows and perform other duties as assigned by the RN
Requirements9
- 1Ohio LPN or LVN license in good standing with current CPR certification
- 2Current licensure and valid driver's license with reliable automobile
- 3Recent TB test
- 43 years of nursing experience preferred
- 5Experience in community/home health or medical/surgical settings preferred
- 6Ability to lift and transfer patients and equipment safely
- 7Effective verbal and written communication and organizational skills
- 8Must be able to travel within ~30 mile radius and be present for on-site visits
- 9Commitment to professional standards and practices
Salary Insight
