What You'll Do8
- 1Assess patient functional status using standardized OT evaluations and document findings in Epic.
- 2Design personalized treatment plans targeting activities of daily living, mobility, and safety.
- 3Implement therapeutic interventions including therapeutic exercise, manual therapy, and adaptive equipment training.
- 4Collaborate with physical therapists, speech therapists, and nursing staff to coordinate holistic care.
- 5Educate patients and caregivers on home modification strategies and fall prevention.
- 6Monitor progress and adjust treatment plans to achieve measurable outcomes within 90 days.
- 7Complete Medicare-compliant documentation and submit timely visit notes.
- 8Communicate with referral sources and physicians to update patient status.
Requirements7
- 1Degree from an ACOTE-accredited occupational therapy program.
- 2Current Ohio OT licensure in good standing.
- 31+ years of home health or community-based therapy experience preferred.
- 4Reliable car and valid driver's license with current insurance.
- 5Ability to travel within the assigned regional territory (Stark, Summit, Medina, or Wayne).
- 6Strong clinical reasoning and documentation skills in an electronic health record system.
- 7Commitment to patient-centered, evidence-based practice.
