Registered Nurse Home Care Ohio
Overview
We seek a Registered Nurse to oversee non-medical home care services via caregiver supervision and client assessments. This role supports Ohio Medicaid Waiver programs by ensuring compliance and quality care.
What You'll Do11
- 1Conduct initial and ongoing in-home assessments of clients receiving personal support or companion services
- 2Perform supervisory visits and wellness checks to ensure caregiver compliance and client safety
- 3Complete up to 25 visits weekly between assessments and supervisory duties
- 4Observe and report client condition changes to supervising RN or care team
- 5Collaborate on creating, amending, and co-signing Individualized Service Plans
- 6Maintain accurate documentation in the agency CRM following Medicaid and state guidelines
- 7Provide field-based coaching to non-clinical caregivers on ADLs infection control safety techniques and client-specific needs
- 8Participate in agency audits quality assurance and client satisfaction initiatives
- 9Serve as liaison between caregivers clients families and clinical leadership
- 10Co-sign documentation related to supervisory visits and care oversight
- 11Assist with onboarding and cross-training of new LPNs or field staff
Requirements8
- 1Active Registered Nursing Degree in Ohio
- 2CPR Certification (BLS for Healthcare Providers)
- 33–5 years experience in home care HCBS or waiver-based services
- 4Strong documentation communication and organizational skills
- 5Ability to work independently manage multiple priorities
- 6Reliable transportation valid driver’s license and auto insurance
- 7Comfortable training non-clinical caregivers
- 8Willingness to travel within and outside Metro Atlanta
Salary Insight
$75 - $80k per year
