Overview

Provide direct patient care and care coordination for Primary Care patients within the Patient Aligned Care Team (PACT) at a Community Based Outpatient Clinic in Sorrento Valley, CA. Work within a multi-disciplinary team to deliver care focused on chronic disease management, patient education, and timely follow-up. This role offers the chance to serve veterans in a dynamic outpatient setting with flexible scheduling and a supportive environment.

What You'll Do9

  • 1Administer medications and treatments to patients with complex nursing problems, documenting responses in the EHR.
  • 2Recognize changes in patient condition and initiate BLS or emergency procedures as needed, including calling RRT or Code Blue.
  • 3Perform bedside Point of Care testing, including nasal swabs and pregnancy testing.
  • 4Educate patients on managing risk factors, medications, and procedures within scope of practice.
  • 5Collaborate with the Interdisciplinary Care Team to coordinate care, including Clinical Video Tele-Health visits.
  • 6Participate in PACT performance activities, such as 2-day post-discharge calls and clinical reminder compliance.
  • 7Mentor and coach new employees, providing in-services on team building or specialty care.
  • 8Ensure patients complete pre-appointment requirements prior to PCP visits.
  • 9Maintain confidentiality and adhere to ethical standards in all patient interactions.

Requirements10

  • 1Active, full, current and unrestricted licensure as a Graduate Licensed Practical or Vocational Nurse (LPN/LVN) in a U.S. State, Territory, or the District of Columbia.
  • 2Graduate of a school of practical or vocational nursing approved by the appropriate State agency or accredited by the NLNAC at the time the program was completed.
  • 31-2 years of current Primary Care (PACT) experience preferred.
  • 4Clinical experience in caring for patients in Ambulatory Care.
  • 5Ability to work collaboratively with an interdisciplinary team.
  • 6Ability to document using electronic medical record (EHR).
  • 7Proficiency in written and spoken English.
  • 8U.S. Citizenship required.
  • 9Must pass pre-employment physical examination and background/security investigation.
  • 10Participation in the seasonal influenza vaccination program is required.
