Adaptive Home Health seeks a Licensed Vocational Nurse (LVN) to deliver skilled home care in Texas. The role expands patient access through reliable field clinicians. What You'll Do6
- 1Deliver skilled nursing visits within LVN scope under RN supervision
- 2Conduct focused assessments and escalate changes to the clinical team
- 3Support medication reconciliation and patient education
- 4Provide wound care and basic monitoring per orders
- 5Document thoroughly in Homecare Homebase
- 6Coordinate with office and interdisciplinary teams for scheduling
Requirements10
- 1Current Texas LVN license (unrestricted)
- 2Comfortable with field-based workflow and escalation judgment
- 3Strong communication and documentation skills
- 4Reliable follow-through on orders and patient goals
- 5Valid Texas driver’s license and daily travel ability
- 6CPR required (BLS)
- 7Prior home health or hospice experience
- 8Familiarity with EMR systems such as HCHB
- 9Wound care exposure preferred
