What You'll Do7
- 1Assist in identifying patient physical psycho social and environmental needs based on documentation and team reports
- 2Participate in planning and implementing care under RN direction following POC guidelines
- 3Apply infection control concepts during patient care activities to prevent disease transmission
- 4Ensure continuity of care through quality patient care delivery
- 5Provide effective communication to patients families and healthcare professionals via clinical notes and case conferences
- 6Monitor assigned cases to meet third party payer requirements
- 7Promote agency philosophy and administrative policies to maintain care quality
Requirements5
- 1Current LVN license valid driver's license valid auto insurance CPR certification
- 2HomeCare HomeBase experience preferred
