Overview
Own delivering skilled nursing visits in patients' homes at scale and within LVN scope under supervision. Collaborate with RN/MD and interdisciplinary teams to coordinate timely visits and follow orders. Leverage charting and scheduling tools to reduce admin burden and protect clinician time. What You'll Do10
- 1Build and perform focused nursing assessments and escalations to RN/MD with objective information
- 2Document thoroughly and on time to ensure defensible records and efficient care
- 3Educate patients and caregivers using teach-back aligned to the plan of care
- 4Support wound care and other skilled tasks per orders and competency
- 5Coordinate with office and interdisciplinary team to ensure on time visits and order adherence
- 6Deliver care within home setting and maintain consistent visit cadence
- 7Assist with medication reconciliation and safety education within scope
- 8Manage field-based workflow with smart routing and predictable schedules
- 9Contribute to charting and scheduling product feedback to reduce documentation burden
- 10Maintain compliance with CPR/First Aid and license requirements

- 1Current Nevada LVN license (unrestricted)
- 2Comfort working in a field-based workflow with escalation judgment
- 3Strong communication and documentation
- 4Reliable follow-through on orders, visit cadence, and patient goals
- 5CPR required (BLS)
- 6Current Nevada driver’s license and comfort with daily travel across assigned territory
