Licensed Vocational Nurse (LVN), Home Health Sacramento
Overview
You will deliver one-on-one nursing care to elderly and adult patients in the Sacramento area, managing 3-7 visits per day. As an LVN, you will own wound care, lab draws, Foley care, and patient education. You will join a stable home care team with 30+ years of local presence. Gain IV Certified status and use Wellsky EMR. This role offers flexible weekday hours and a rotating on-call schedule.
What You'll Do8
- 1Provide one-on-one nursing services in patients' homes, averaging 3-7 visits per day for elderly and adult populations.
- 2Deliver wound care, lab draws, Foley care, and ostomy care per the plan of care.
- 3Educate patients and families on disease process, medications, POC, and home care procedures.
- 4Monitor patient progress using observation, assessment, and evaluation skills, and report adverse findings to physicians and RNs.
- 5Document each visit in Wellsky EMR within compliance standards.
- 6Follow HIPAA regulations and infection control policies in and out of the office.
- 7Collaborate with interdisciplinary team and office staff to coordinate patient care.
- 8Complete IV Certified procedures and maintain certification.
Requirements8
- 1LVN license in California
- 2IV Certified
- 31+ years home health or acute care experience
- 4Current CPR certification
- 5Valid driver's license and reliable transportation
- 6Knowledge of EMR systems, specifically Wellsky preferred
- 7Ability to lift or transfer patients up to 50 pounds
- 8Flexible schedule for Monday-Friday and rotating on-call every 4th week
