Overview
Own LVN clinical care within a high-touch recuperative center supporting individuals experiencing homelessness. Scale impact across medical, behavioral health, and social services, coordinating with case managers and CNAs to prevent hospital readmission. What You'll Do10
- 1Build daily rounds, assess health and vital signs, and document changes to the supervising nurse.
- 2Design weekly medication management processes and support adherence and refills with patient education.
- 3Lead client outreach visits and telephonic support to maintain engagement and track progress.
- 4Ship blood glucose monitoring, insulin instruction, and self-care training to clients.
- 5Own basic nursing care tasks such as wound care, dressing changes, and hygiene support.
- 6Drive health education on conditions, medications, and self-care to achieve health goals.
- 7Collaborate with CNAs, interns, and volunteers to support nursing programs and trainings.
- 8Coordinate scheduling of medical appointments and transportation for clients as needed.
- 9Engage in case reviews and multidisciplinary meetings to improve quality and discharge planning.
- 10Maintain compliant documentation and ensure supply areas are stocked and prepared.
Requirements6
- 1Valid and unrestricted LVN license in California
- 2Minimum of 1 year of clinical nursing experience
- 3Valid CPR, BLS and First Aid certificates within three months of hire
- 4Experience in behavioral health, community health, or residential care preferred
- 5Experience with clients who are medically fragile or experiencing homelessness preferred
- 6Bilingual Spanish/English preferred
