Overview
As a Licensed Physical Therapist at EmpowerMe Wellness, you own the therapy journey for senior residents across Dallas, Texas, within our EmpowerMe 360 model of care. You deliver therapy, wellness, and care coordination in an onsite setting, partnering with a collaborative team of therapists, wellness professionals, and medical providers. You build trusted relationships with residents, helping them reduce fall risk, master daily tasks, and maintain independence. Your role stands out through predictable schedules, no constant driving, and a culture that celebrates therapists through leadership paths and specialty programs.
What You'll Do8
- 1Deliver physical therapy, occupational therapy, or speech therapy services to older adults in senior living communities, focusing on functional mobility and independence.
- 2Conduct comprehensive assessments and create personalized care plans using evidence-based interventions, adjusting them based on resident progress.
- 3Coordinate care through wellness check-ins, caregiver communication, and telehealth support to ensure seamless integration of services.
- 4Maintain professional and ethical standards, adhering to company initiatives and regulatory requirements for PT practice.
- 5Contribute to program development and quality improvement by sharing insights with the clinical team.
- 6Help residents maximize independence, confidence, and satisfaction through targeted therapeutic exercises and education.
- 7Collaborate with medical providers and wellness staff to align with the EmpowerMe 360 integrated care model.
- 8Document resident progress accurately and timely, using EMR systems to track outcomes.
Requirements5
- 1Current professional license in PT, PTA, OT, COTA, or SLP in the state of Texas.
- 21+ years of experience in geriatric rehabilitation or a related setting, with a passion for empowering seniors.
- 3Strong clinical skills in assessment, treatment planning, and intervention delivery, with a collaborative spirit.
- 4Commitment to teamwork and community visibility, as you will interact with residents, families, and staff daily.
- 5Ability to work onsite in Dallas, Texas, with a flexible schedule that supports work-life balance.
