What You'll Do10
- 1Build and deliver individual and group counseling for older adults LCMHC credential in North Carolina
- 2Develop mental health assessments and care plans to improve client well being
- 3Collaborate with families, caregivers, and facility staff to coordinate care
- 4Maintain clinical documentation in accordance with ethical standards HIPAA and regulatory guidelines
- 5Participate in team meetings, training, and program development to scale services
- 6Contribute ideas to shape the evolving care model and care delivery
- 7Support hybrid care through in person and telehealth sessions
- 8Engage in trauma informed approaches and chronic illness considerations when relevant
- 9Support equity, dignity, and whole person wellness in every interaction
- 10Drive quality improvements and share learnings with the team

- 1Licensed Clinical Mental Health Counselor (LCMHC) in North Carolina
- 2Experience with older adults, trauma informed care, or chronic illness preferred
- 3Strong communication and relationship building skills
