Overview
The Clinical Mental Health Professional at Intermountain Health Alta View Hospital leads psychosocial assessments and develops care plans within a multidisciplinary team. This role focuses on providing family and individual treatment while collaborating with patients, families, and external providers. What You'll Do9
- 1Diagnose and treat mental emotional and behavioral disorders while modifying care plans based on patient status
- 2Refer patients to community services and specialists to enhance overall well-being
- 3Consult with multidisciplinary teams to create discharge plans ensuring seamless care transitions
- 4Prioritize daily workload by addressing new current and discharging patient needs
- 5Conduct psychosocial evaluations and provide crisis intervention and bereavement support
- 6Coordinate with patient care teams and external agencies to resolve care delays
- 7Support staff through debriefing after adverse events via Employee Assistance Program
- 8Maintain accurate documentation using electronic health records and verify credentials through staff office
- 9Provide guidance and mentorship to colleagues facing challenging situations
Requirements9
- 1Master’s degree in social work or related field from accredited institution
- 2Current state licensure as LCSW LMFT CMHC/LPC/LCPC or Psychologist
- 3Excellent written and verbal communication skills
- 4Ability to operate medical monitors and manipulate complex equipment with precision
- 5Physical stamina for lifting up to heavy items and transferring patients
- 6Experience with behavioral health interventions and patient coordination
- 7Dual licensure with state as LAC Montana for Montana residents
- 8Strong conflict resolution abilities and familiarity with EHR systems
- 9Commitment to collaborative teamwork in fast paced clinical environments
