What You'll Do8
- 1Provide hands-on personal care, including bathing, dressing, and grooming, to hospice patients in their homes or facilities, ensuring comfort and dignity.
- 2Assist with repositioning and mobility, using proper techniques to lift or transfer patients up to 200 lbs with assistance, reducing fall risk.
- 3Monitor patient comfort levels and respond to needs promptly, reporting any changes to the care team to adjust care plans.
- 4Support daily living activities such as feeding, toileting, and range-of-motion exercises, maintaining patient safety and infection control protocols.
- 5Adapt to varying home environments, including temperature changes and moderate noise, while upholding safety and hygiene standards.
- 6Drive to patient locations across the coverage area (e.g., 33054, 33167) using a reliable vehicle, managing travel time effectively.
- 7Document care provided and observations accurately, following HIPAA guidelines to ensure continuity of care.
- 8Collaborate with nurses and family members to deliver compassionate end-of-life care, respecting patient preferences and cultural needs.
Requirements8
- 1Certified by the state as a nurse's aide, with completion of required training and a passing score of 85% or higher on the agency's competency exam.
- 2CPR certification required for direct patient care in Georgia and Washington, but preferred for all states.
- 3Must be listed on the state aide registry with a clear record and eligible to provide care in homes, nursing facilities, or hospitals.
- 4Requires a reliable vehicle, valid driver's license, and state-required auto insurance.
- 5Ability to perform physical activities including walking, standing, reaching, and lifting up to 200 lbs with assistance.
- 6Comfortable working in home and facility environments with varying conditions such as temperature changes and moderate noise.
- 720 hours per week availability, with a schedule of Sunday-Thursday or Tuesday-Saturday.
- 8Willingness to travel within the Moon Team coverage area, including zip codes 33054, 33167, 33168, 33147, 33142, 33125, 33128, 33136, and 33127.
