What You'll Do8
- 1Conduct initial OASIS assessments and develop patient-centered goals aligned with the physician's plan of care.
- 2Treat patients using evidence-based physical therapy techniques, including therapeutic exercise, manual therapy, and durable medical equipment training.
- 3Remove barriers to patient progress by adapting treatments and coordinating with the care team on each visit.
- 4Document all patient reactions and changes in condition, reporting findings to the physician and care manager.
- 5Instruct patients, families, and home health aides in therapy techniques, including the use of prosthetic and orthotic devices.
- 6Educate patients on injury prevention and home exercise programs to build independence and reduce readmissions.
- 7Complete non-visit discharges and transfers in the EMR, earning $5.00 per non-visit discharge and $10.00 per transfer.
- 8Participate in office meetings and required in-service training, maintaining compliance with agency policies.
Requirements7
- 1Active Michigan Physical Therapy license
- 21+ years of home health experience preferred, but new graduates with strong clinical rotations considered
- 3Reliable transportation and valid driver's license with auto insurance in your name
- 4Ability to navigate patient homes and travel up to 50 miles per day for visits
- 5Completion of required in-service programs and orientation, paid at $30.00 per hour
- 6Strong clinical judgment for OASIS documentation and plan of care development
- 7Comfort with electronic medical records and mobile documentation tools
