Physical Therapist - Home Health (Indianapolis, IN)
Overview
You will own the full therapy arc for home health patients across Marion, Hamilton, and adjacent counties, directing evaluations, OASIS assessments, and treatment plans under physician orders. You'll join Redcrest Home Health as part of Alternate Solutions Health Network, a team that treats patients like family. Expect a mix of independent fieldwork and collaborative case conferences with physicians, PTAs, and interdisciplinary staff. This role offers a $10,000 sign-on bonus and a consistent Monday-Friday schedule, with travel built into your daily routine.
What You'll Do8
- 1Evaluate patients at home, complete OASIS assessments, and craft individualized plans of care.
- 2Deliver skilled therapy interventions and adjust treatment plans based on measured progress.
- 3Educate patients and caregivers on mobility, fall prevention, and assistive devices.
- 4Coordinate with physicians and the care team, including PTAs, to keep treatment aligned.
- 5Supervise physical therapist assistants and complete required supervisory visits.
- 6Document evaluations and progress notes within agency timeframes, meeting Medicare rules.
- 7Participate in case conferences, on-call rotation, and team meetings.
- 8Travel to patient homes across assigned territories.
Requirements5
- 1Current Physical Therapist license in Indiana.
- 21+ year of acute care experience; home health preferred.
- 3Knowledge of OASIS and Medicare home health regulations.
- 4Valid driver's license, reliable car, and auto insurance.
- 5Physical ability to lift 50 lbs, stand, walk, and bend during home visits.
