- 4Drive to patient locations across Chicago and Lincolnwood, spending ~20% of your day in transit, optimizing routes to maximize visit efficiency.
- 5Communicate patient updates and care adjustments to referring physicians and family members, ensuring alignment with care goals.
- 6Mentor new clinicians during onboarding, sharing best practices from your 1+ year of healthcare experience.
Requirements7
- 1Bachelor's or Master's degree in Physical Therapy from an accredited college.
- 2Licensed Physical Therapist in the state of Illinois.
- 31+ year of healthcare experience, preferably in home health or outpatient settings.
- 4Valid driver's license and ability to spend ~20% of your day driving between patient locations.
- 5Proven ability to meet critical deadlines for clinical charting.
- 6Comfort with tablets, smartphones, and EMR software for documentation.
- 7Self-management skills to schedule visits and balance caseload independently.
