Overview
Fulltime therapist at CenterWell Home Health in Chicago providing rehab services for homebound patients in North Cook County. You will plan and administer PT treatments, assess patient strengths, develop rehab programs, train PTA staff, document progress, and report to physicians and families. This role is part of Humana's Tuberculosis screening program requiring TB screening. Requires current PT license CPR certification collaboration skills. Compensation $65-$91 per visit. Must drive a personal vehicle with liability insurance.
What You'll Do9
- 1Plan and administer prescribed physical therapy treatments onsite to improve patient function
- 2Assess and document patient strengths to support physician evaluations
- 3Develop rehabilitation programs aligned with physician plans of treatment
- 4Train and supervise PTA and Home Health Aide staff
- 5Document patient observations interventions and evaluations promptly
- 6Report patient progress to physicians clinical managers and families
- 7Submit required evaluations plans and discharge summaries
- 8Participate in Humana driver safety program with personal vehicle liability insurance
- 9Maintain confidentiality and adhere to privacy regulations
Requirements10
- 1Current unrestricted Physical Therapy license
- 2CPR certification
- 3Experience collaborating with healthcare teams
- 4Valid state driver's license and personal vehicle liability insurance
- 5One year of PT experience preferred
- 6Ability to operate heavy equipment safely
- 7Strong communication skills for patient education
- 8Knowledge of TB screening protocols
- 9Familiarity with electronic health record systems
