What You'll Do9
- 1Complete comprehensive evaluations and identify functional needs
- 2Develop individualized plans of care and treatment goals
- 3Provide evidence-based interventions and monitor patient progress
- 4Reassess patients and modify treatment plans as clinically appropriate
- 5Deliver skilled therapy including therapeutic exercise and gait training
- 6Educate patients and caregivers on fall prevention and home exercise programs
- 7Maintain accurate documentation and collaborate with physicians
- 8Work independently while staying connected to the interdisciplinary team
- 9Drive patient outcomes through proactive rehabilitation strategies
Requirements10
- 1Current Michigan PT or PTA license in good standing
- 2Degree from accredited PT or PTA program
- 3Experience in home health settings preferred
- 4Strong clinical judgment and problem-solving abilities
- 5Excellent communication and patient education skills
- 6Reliable transportation and valid driver's license
- 7Familiarity with Medicare CMS documentation
- 85+ years of relevant clinical experience
- 9Proficiency with electronic health records
- 10Certification in home health certification preferred
