What You'll Do11
- 1Receive and document referrals into the Care Management platform
- 2Conduct outreach and schedule members for Care Managers
- 3Verify eligibility and demographic details for all applicants
- 4Assist with educational material distribution and referral coordination
- 5Provide culturally appropriate health education and social support
- 6Advocate for members and identify care gaps for outreach
- 7Submit supplemental data to health plans for gap closure documentation
- 8Schedule medical appointments and transportation as needed
- 9Pull and analyze Care Gap Recommended Actions High Risk reports
- 10Address Social Determinants of Health through resource navigation
- 11Maintain HIPAA compliance and adhere to privacy policies
Requirements10
- 1High school diploma or GED
- 2Minimum 2 years healthcare experience
- 3Preferred 4-year degree in health-related field
- 4Valid driver's license with auto liability insurance
- 5Proficiency in Microsoft Office and internet use
- 6Knowledge of state/federal benefits systems
- 7Strong communication and motivational interviewing skills
- 8Ability to handle diverse cultural and linguistic backgrounds
