Join a collaborative team delivering Family Connections and Nurturing Parenting while coordinating resources and services. This role blends field visits with documentation duties, requiring flexibility for evenings and weekends. What You'll Do5
- 1Build and maintain weekly and monthly contact with families to address needs using evidence based curriculum and community resources
- 2Meet contract requirements, complete progress notes, and manage tasks to ensure timelines are met
- 3Collaborate with Team Lead to guide families toward positive outcomes and sustainable behavioral changes
- 4Deliver short term, time limited coordination of services and resources based on ADCS identified stressors
- 5Provide in home case management, navigation of services, and support to stabilize and preserve home environments
Requirements5
- 1Bachelor’s degree or higher in a related field
- 2Experience working with family dynamics and diverse backgrounds
- 3Knowledge of child abuse, neglect, child development, and ADCS regulations
- 4Strong verbal and written communication skills with ability to document accurately
- 5Must have reliable transportation and pass background checks and fingerprint clearance
