Family Nurse Practitioner, Virtual Wellness Visits
Overview
You own virtual wellness visits for patients in the New York area, delivering care through HIPAA-secure platforms and coordinating with in-home Sprinters. You join a team of clinicians and technologists building a mobile health service that increases access and lowers costs. This role offers a 3-week paid sabbatical in Q1 and the chance to shape a new care model. You work autonomously but stay connected to a collaborative team, making a direct impact on patient outcomes.
What You'll Do7
- 1Review your daily case load and prep your technology, including laptops and mobile devices, to ensure seamless virtual visits.
- 2Navigate your schedule using modern scheduling tools and partner with in-home Sprinters to coordinate patient care.
- 3Conduct thorough virtual health risk assessments, medication reviews, and cognitive screenings during wellness visits.
- 4Provide patient education on health and well-being, empowering them with actionable insights.
- 5Collaborate with Sprinters to evaluate vital signs, arrange blood draws, and address specific patient needs.
- 6Manage comprehensive care coordination, including preventive interventions, medication management, and specialist referrals.
- 7Document all findings accurately in HIPAA-compliant systems after each visit.
Requirements7
- 1Board Certified as a Family Nurse Practitioner (FNP-C or FNP-BC).
- 2Active New York State Family Nurse Practitioner license.
- 32+ years conducting wellness visits or similar primary care services.
- 4Proficiency with telehealth platforms and HIPAA-secure messaging apps.
- 5Strong clinical judgment and ability to work independently.
- 6Excellent written and verbal communication skills.
- 7Pass a national background check and valid license verification.
