What You'll Do6
- 1Provide comprehensive outpatient primary care to pediatric, adult, and geriatric patients, including physical exams, health assessments, and preventive screenings.
- 2Diagnose and manage acute and chronic conditions such as hypertension, diabetes, asthma, and hyperlipidemia, ordering and interpreting diagnostic tests and labs.
- 3Prescribe medications and develop individualized treatment plans per New York state regulations, adjusting based on patient response.
- 4Educate patients on disease prevention, wellness, and lifestyle modifications, empowering self-management.
- 5Collaborate with physicians and specialists to ensure coordinated, high-quality care, participating in quality improvement initiatives and team meetings.
- 6Maintain accurate, timely documentation in the EHR, ensuring compliance and continuity of care.
Requirements5
- 1Master's or Doctorate degree in Nursing from an accredited Nurse Practitioner program.
- 2Board Certified as a Family Nurse Practitioner (FNP) with active or eligible New York licensure.
- 3DEA registration or eligibility required.
- 4Prior experience in family practice or primary care preferred; new graduates with strong clinical training encouraged.
- 5Excellent communication and collaboration skills, with a patient-centered approach.
