What You'll Do7
- 1Deliver comprehensive primary care, from preventive exams to acute illness management and chronic disease care.
- 2Perform physical exams and order, interpret, and evaluate diagnostic tests.
- 3Design and adjust individualized treatment plans for each patient.
- 4Educate patients on prevention, health maintenance, and lifestyle changes.
- 5Coordinate with physicians and specialists to ensure integrated care.
- 6Document every encounter accurately in EHR within 24 hours.
- 7Drive quality improvement and population health initiatives from day one.
Requirements7
- 1MSN or DNP from an accredited Nurse Practitioner program.
- 2Active or eligible New York State FNP license and national certification.
- 32+ years of primary care or family medicine experience preferred.
- 4Proficiency with EHR systems and telemedicine platforms.
- 5BLS certification required.
- 6Strong clinical judgment and ability to work independently.
