Family Nurse Practitioner, Primary Care & Chronic Illness Management
Overview
You will own patient care for a diverse caseload of adults and children with acute and chronic conditions at a busy New York City practice, managing 15-20 patients daily. You will diagnose, treat, and coordinate care using EClinicalWorks EHR, collaborating with a tight-knit clinical team of physicians and support staff. This role offers both part-time and full-time schedules, with direct patient impact from day one.
What You'll Do7
- 1Own the full patient visit cycle: assess symptoms, order and interpret labs and imaging, and deliver diagnoses with treatment plans in EClinicalWorks.
- 2Prescribe appropriate medications and manage refills, adjusting regimens based on patient response and lab results.
- 3Refer patients to specialists and coordinate follow-up care, ensuring seamless transitions and clear documentation.
- 4Complete all charting and clinical notes within 24 hours, maintaining HIPAA compliance and accurate records.
- 5Lead patient education on chronic disease management, preventive care, and medication adherence, tailoring advice to each patient's needs.
- 6Collaborate with the care team to optimize clinical workflows and reduce wait times, using EClinicalWorks templates and alerts.
- 7Drive quality metrics by meeting HEDIS standards for preventive screenings and immunizations in your patient panel.
Requirements7
- 12+ years clinical experience as a Family Nurse Practitioner in an outpatient setting.
- 2Active FNP license and certification in New York State, with prescriptive authority.
- 3Graduation from an accredited Nurse Practitioner program with a Master's or Doctorate.
- 4Strong clinical judgment in diagnosing acute and chronic conditions across all ages.
- 5Proficiency with EClinicalWorks EHR or similar systems, with the ability to document efficiently.
- 6Excellent interpersonal skills for patient communication and team collaboration.
- 7Ability to manage a high-volume schedule while maintaining accuracy and empathy.
