What You'll Do10

  • 1Build and provide comprehensive primary care including preventive, acute, and chronic condition management for a dedicated patient panel
  • 2Assess patients through histories, examinations, diagnostics, and implement treatment plans with emphasis on trauma-informed care
  • 3Deliver culturally responsive, linguistically appropriate care and education tailored to individuals and communities
  • 4Document encounters accurately in the EHR and review external records to inform decisions
  • 5Coordinate referrals and services including behavioral health, nursing, case management and specialty care
  • 6Collaborate with interdisciplinary teams in regular meetings and shared care planning to support whole-person care
  • 7Provide urgent and after-hours care as needed and participate in rotating on-call coverage
  • 8Participate in continuing education and staff training including supervision of learners
  • 9Contribute to clinical quality improvement and risk management activities in collaboration with the clinical team
  • 10Meet with supervisor regularly to review performance, documentation, and patient feedback

Requirements7

  • 1Master’s degree in Nursing
  • 2Board Certification as a Family Nurse Practitioner
  • 3Active or eligible licensure in the District of Columbia and Maryland
  • 42+ years experience providing primary care as a Nurse Practitioner
  • 5Experience using electronic health records in ambulatory settings
  • 6Preferred experience in FQHCs or community health settings
  • 7Spanish proficiency is a plus, not required
