Overview

You will own primary care for a Medicaid and Medicare population at our Portland integrated clinic, part of Oregon Integrated Health. You will practice alongside Family Nurse Practitioners, Naturopathic Physicians, and behavioral health clinicians in a Tier 5 Patient-Centered Medical Home. You will get 45-minute new patient visits, 30-minute follow-ups, and dedicated Medical Assistants and Patient Health Navigators to handle rooming, referrals, and care coordination. This role also qualifies for the Oregon Loan Repayment Program and offers a $8,000 sign-on bonus.

What You'll Do8

  • 1Provide comprehensive primary care to patients of all ages, including physical exams, diagnostic tests, and treatment plans.
  • 2Manage chronic conditions and acute illnesses, with an emphasis on preventive care and patient education.
  • 3Perform telemedicine consultations for patients who are homebound or lack reliable access to technology.
  • 4Collaborate with an integrated team of behavioral health, chiropractic, acupuncture, and other specialists to coordinate whole-person care.
  • 5Order and interpret lab work, imaging, and other diagnostics, then communicate results and next steps to patients.
  • 6Document all patient encounters accurately in EHR within clinic policy and compliance standards.
  • 7Refer patients to community resources and follow up on ED visits, complex care cases, and quality metrics.
  • 8Maintain a full schedule with same-day appointments available for urgent needs, ensuring timely access.

Requirements7

  • 11+ years of experience in urgent care, family practice, or emergency settings.
  • 2Graduate of an Accredited Medical School with a current Family Nurse Practitioner license.
  • 3Active Oregon State Medical License and full prescriptive authority.
  • 4Current DEA Certificate and active National Provider Identifier (NPI).
  • 5Comfortable using electronic health records (EHR) and telehealth platforms.
  • 6Ability to work flexible hours, including some evenings or weekends as needed.
  • 7Strong communication skills for collaborating with a multidisciplinary team and diverse patient population.
