Overview
Owns comprehensive primary care delivery for patients across all ages in an outpatient clinic setting. Leads evidence based assessments and treatment plans that improve health outcomes at Buena Park and Bakersfield sites. Collaborates with the multidisciplinary team to promote preventive care and manage acute and chronic conditions. What You'll Do10
- 1Build patient assessment workflows including vital signs history and exams to determine health status
- 2Diagnose and treat acute illnesses and chronic conditions using evidence based practices
- 3Prescribe medications in compliance with state licensing and practice guidelines
- 4Manage electronic records in EHR systems ensuring accurate and complete documentation
- 5Provide patient education and care coordination across the care team
- 6Collaborate with physicians and staff to optimize clinic operations and patient flow
- 7Support preventive care initiatives and chronic disease management programs
- 8Engage with diverse patient populations including pediatrics and geriatrics
- 9Document encounters for quality assurance and continuity of care
- 10Participate in performance improvement and continuing education activities
Requirements7
- 1Valid state licensure as a Family Nurse Practitioner with current certification from the American Association of Nurse Practitioners
- 2Bilingual in English and Spanish
- 3Experience in primary care settings with proficiency in a medical office environment
- 4Strong clinical skills including physical examinations and diagnostic evaluation
- 5Familiarity with EMR/EHR systems and medical documentation
- 6Experience serving diverse populations including pediatrics, geriatrics, and individuals with disabilities
- 7Ability to manage outpatient clinic responsibilities and collaborate with a multidisciplinary team
