Family Advocate, Early Childhood Education & Case Management
Overview
Own intake, enrollment, and case management for 40 families in Atlanta's only shelter for homeless families with newborns. Partner with families to set goals, connect them to housing, medical care, and early education, and build community referral networks. This role drives stability through HMIS, GoEngage, and Penelope data systems, working with a collaborative team. You will shape family success with measurable outcomes from day one.
What You'll Do10
- 1Complete eligibility and enrollment for families, collecting required paperwork and entering data into HMIS, GoEngage, and Penelope within the first 30 days.
- 2Carry a caseload of 40 families, conducting needs assessments to identify current and ongoing needs.
- 3Meet with families weekly or monthly to set short- and long-term goals, providing conflict resolution, problem solving, and life skills support.
- 4Coordinate with external housing case managers, visiting placements and assisting with housing applications and transitions to permanent housing.
- 5Partner with other Family Advocates to facilitate parent, community, and psycho-educational groups.
- 6Maintain accurate client records, documenting service objectives, interventions, and outcomes per Federal, State, and County guidelines.
- 7Assist with grant reporting, tracking deliverables, and data clean-up in assigned software systems.
- 8Build and maintain community partnerships, creating a referral base and following up to ensure services are received.
- 9Participate in interagency collaborations and community groups as requested.
- 10Model professionalism, empathy, and active listening in all interactions, contributing to broader organizational goals.
Requirements10
- 1Bachelor’s Degree in Social Work or related field from an accredited college or university; Master’s degree preferred.
- 21+ years of experience working with families in need of support; 2+ years preferred.
- 3Proven ability to form positive connections with diverse families, children, and volunteers.
- 4Strong organizational and strategic planning skills, with capability to multitask independently.
- 5Excellent verbal and written communication skills.
- 6Proficient in Microsoft Office and complex software systems like HMIS, GoEngage, and Penelope.
- 7Physical ability to lift up to 30 pounds, with prolonged sitting and typing.
- 8Proof of Tuberculosis (TB) test clearance prior to employment, and compliance with other health screenings.
- 9Availability for in-office work 40 hours/week, Monday through Friday, with occasional evenings/weekends for events.
- 10Commitment to professionalism, including attire, timeliness, accountability, and lifelong learning.
