Family Advocacy Nurse, In-Home Maternal & Child Health (NPSP)
Overview
The FAN will own in-home nursing and prevention services for military families with children under four, focusing on preventing maltreatment. You will lead home visits, case management, and family education within a multidisciplinary Family Advocacy Program team across CONUS locations. This role emphasizes field work, maternal and child health, and collaborative care with military healthcare partners. What sets this role apart is direct, in-home support with an emphasis on prevention and community-based family health.
What You'll Do5
- 1Build and deliver comprehensive, home-based nursing and prevention services through the NPSP.
- 2Lead home visits, assess needs, and coordinate care per maltreatment treatment plans.
- 3Manage NPSP cases including assessment, education, monitoring, and follow-up.
- 4Provide anticipatory guidance on child development, maternal health, nutrition, parenting, and family wellness.
- 5Partner with FAP and MTF teams to ensure compliance with nursing standards and program requirements.
Requirements5
- 1Bachelor`s degree in Nursing (BSN) from an accredited institution
- 2U.S. Registered Nurse (RN) with an unrestricted license
- 3Maintains licensing standards and MTF credentials
- 4American Heart Association BLS or Red Cross CPR/BLS certification
- 5Minimum two years in Community Health, Maternal/Child Health, or related areas
