What You'll Do8
- 1Assist residents to and from medical appointments in and around the Rogers Park community, ensuring timely arrival and departure.
- 2Verify each resident's identity before transport to match appointment records and prevent errors.
- 3Check that residents have all necessary equipment and supplies for their appointments, such as wheelchairs, oxygen, or medication.
- 4Help residents enter and exit vehicles safely, using proper lifting or transfer techniques as needed.
- 5Carry resident items during transport, including personal belongings and medical documents.
- 6Coordinate with facility staff and appointment providers to confirm transport methods and schedules, using CNA skills to monitor resident condition.
- 7Report any changes in resident health or behavior to nursing staff immediately, contributing to the care plan.
- 8Maintain a clean and organized transport area, ensuring wheelchairs and other mobility aids are in good condition.
Requirements8
- 1High school diploma or equivalent, with a valid, unencumbered State of Illinois Nursing Assistant Certification (C.N.A.).
- 2One (1) year of experience in a Customer Service role is preferred, but entry-level candidates with the right attitude are considered.
- 3Current CPR certification or ability to obtain within 90 days of hire.
- 4Valid driver's license and clean driving record for operating facility transport vehicles.
- 5Ability to lift and transfer residents up to 50 pounds using proper body mechanics.
- 6Strong attention to detail for verifying resident identification and appointment paperwork.
- 7Effective communication skills to interact with residents, families, and medical staff.
- 8Flexibility to work varied shifts, including weekends and holidays, based on appointment schedules.
