Community Health Worker, Bilingual Spanish & Patient Navigation
Overview
The role owns patient navigation, care coordination, and connecting patients with healthcare and community resources at a leading Houston primary care group. You will work onsite in a collaborative team with doctors, nurses, and medical assistants to remove barriers to care. This position emphasizes compassionate, patient‑focused support and meaningful impact on families and communities.
What You'll Do10
- 1Build and manage a caseload of patients to coordinate care and follow-up within a 90‑day window
- 2Connect patients with healthcare, financial, and community resources to improve access
- 3Assist with applying for benefits such as Medicare, Medicaid, SNAP, and prescription assistance
- 4Schedule appointments, referrals, and follow‑ups to ensure timely care
- 5Educate patients on managing chronic conditions and staying adherent to plans
- 6Communicate with patients by phone, in person, text, and video to maintain engagement
- 7Collaborate with clinicians and care teams to document interactions and care plans in the EMR
- 8Identify and help overcome barriers to care including transportation, language, and eligibility issues
- 9Track outcomes and update patient status to support continuous care improvement
- 10Remain compliant with privacy and documentation standards to maintain patient trust and safety
Requirements10
- 1Bilingual in English and Spanish
- 2High School Diploma or GED
- 34-5+ years of healthcare or relevant experience
- 4Experience in case management, patient navigation, care coordination, or similar role
- 5Experience connecting patients with community resources and government programs
- 6Experience assisting with benefit applications such as SNAP, LIS, PAP, or prescription assistance
- 7EMR experience
- 8Strong communication, organization, and time-management skills
- 9Compassionate and patient-focused approach
- 10Ability to work effectively with people from diverse backgrounds
