What You'll Do11
- 1Manage care transitions for patients from ED inpatient SNF specialty to primary care 20%
- 2Address barriers to facilitate care coordination improve patient experience and retention-in-care 10%
- 3Assemble clinical background information and present relevant details to teams and insurers 10%
- 4Contact organizations and insurance companies to secure prior approvals and share medical information 10%
- 5Act as system navigator and point of contact handling patient questions and advocating for care needs 5%
- 6Identify cultural resources and serve as liaison between families and care providers 10%
- 7Implement solutions for scheduling transportation and interdisciplinary communication 10%
- 8Track referral processes ensuring appointments and follow-ups while addressing barriers 5%
- 9Use motivational interviewing and teach back techniques to enhance patient understanding 5%
- 10Maintain detailed documentation on patient referrals and care coordination outcomes 10%
- 11Collaborate with teams to schedule appointments and manage patient care plans 10%
Requirements10
- 1High School Diploma or GED required
- 21-3 years clinical care experience
- 3Knowledge of community resources for reentry in Denver Health's integrated system
- 4Strong oral and written communication skills
- 5Ability to work with disadvantaged populations and those facing homelessness
- 6Intermediate knowledge of Denver Metro area community resources
- 7Bilingual English Spanish preferred
- 8Familiarity with Epic and Microsoft Office applications
- 9Shift availability in United States of America
- 10Regular salary range $24.56 - $34.38 per hour
