What You'll Do6

  • 1Complete Mental Health Assessments for clubhouse members and document findings in the County EHR
  • 2Consult on care plan goals and action steps
  • 3Review and approve care plans within established timeframes
  • 4Collaborate with clubhouse staff to address individual member needs
  • 5Promote evidence‑based practices such as motivational interviewing housing first and harm reduction
  • 6Support the implementation of recovery language and culturally responsive care

Requirements5

  • 1California LCSW license or equivalent
  • 2Minimum one year experience supporting adults with mental health conditions
  • 3Strong knowledge of computer software including Outlook Word and Excel
  • 4Ability to communicate effectively verbally and in writing
  • 5Commitment to confidentiality and exceptional interpersonal skills
