Overview

What You'll Do6
- 1Complete Mental Health Assessments for clubhouse members and document findings in the County EHR
- 2Consult on care plan goals and action steps
- 3Review and approve care plans within established timeframes
- 4Collaborate with clubhouse staff to address individual member needs
- 5Promote evidence‑based practices such as motivational interviewing housing first and harm reduction
- 6Support the implementation of recovery language and culturally responsive care
Requirements5
- 1California LCSW license or equivalent
- 2Minimum one year experience supporting adults with mental health conditions
- 3Strong knowledge of computer software including Outlook Word and Excel
- 4Ability to communicate effectively verbally and in writing
- 5Commitment to confidentiality and exceptional interpersonal skills
