Case Manager Administrative Assistant, HCS TxHmL Programs
Overview
Coordinate and monitor services for individuals in HCS and TxHmL programs at kADTEMS LLC. Work directly with clients, guardians, Service Coordinators, residential staff, nurses, and behavioral providers to ensure services are delivered and documented. Manage IPC, PDP, Implementation Plans, and assessments. Maintain audit-ready files and support HHSC audits and surveys. This role requires strong organizational skills and a commitment to client confidentiality and rights.
What You'll Do10
- 1Coordinate and monitor services for assigned HCS/TxHmL individuals, ensuring IPC, PDP, and Implementation Plans are current.
- 2Develop and update Implementation Plans and individual goals with input from the service planning team.
- 3Attend IDT/SPT meetings, annual reviews, and IPC revision meetings, and follow up on action items.
- 4Monitor medical appointments, physician orders, medication changes, and referrals, and follow up as needed.
- 5Review progress notes, service delivery logs, medication documentation, and behavioral records for accuracy.
- 6Maintain organized, accurate, and audit-ready client files, ensuring all documentation is current.
- 7Conduct routine follow-up with individuals, staff, and residential homes regarding health, safety, and services.
- 8Ensure staff have current client-specific plans and required training before providing services.
- 9Report incidents, hospitalizations, and significant changes to appropriate parties promptly.
- 10Support HHSC audits, surveys, record requests, admissions, transfers, and discharges.
Requirements7
- 1Experience in HCS, TxHmL, IDD, case management, or healthcare administration preferred.
- 2Strong organizational and documentation skills, with attention to detail.
- 3Computer proficiency in Microsoft Office and record-tracking systems.
- 4Ability to manage multiple clients, appointments, deadlines, and records.
- 5Maintain confidentiality and professional boundaries in all interactions.
- 6Valid Texas driver's license and reliable transportation preferred.
- 7Must pass background checks and complete agency training.
